Before Flight:

 

1. Open Link: https://hdf.baguio.gov.ph

2. Create your own account,

3. Choose “HDF” or “HEALTH DECLARATION FORM” 4. Choose “New Registration”

5. Fill up Necessary Details

6. Ensure everyone already has a Reference Number (most important step)

7. Secure Medical Certificate from AFGH

8. Coordinate with Tog1 for shuttle to Baguio Triage Area (Convention Center)

 

Upon Arrival at Baguio

 

1. Initial triage at Tog 1

2. Shuttle to Baguio Triage Area

3. Prepare reference Number from created health declaration form

4. Depende sa baguio triage kung mag antigen test, payment c/o TOG1 in coordination with A3 or party.
